Palliative Care Plus is a bridge program designed for the home-bound patient coping with a life-threatening illness who wants curative treatment or is considering hospice services.
This program manages a patient’s pain and symptoms while providing support and education to help them and their families make informed decisions. We help facilitate communications between the patient, family, attending physician and other care providers. Palliative Care Plus (Bridge) Program offers:
